NEW PATIENT INFORMATION

FORMS REQUIRING COMPLETION

New patients are required to provide contact details and medical history. To save time, you can request a copy of the form via email or SMS before your appointment. Simply complete it and bring it along to your visit.

PAYMENT METHODS

We accept the following payment options:

  • Cash
  • Bank Card
  • EFTPOS
  • Visa / MasterCard
  • HICAPS (on-the-spot health insurance claims)
  • Invoicing for NDIS and HCP providers

ACCOUNT PAYMENTS

All accounts must be settled at the time of consultation. Payments can be made via cash, cheque, or EFTPOS. We accept all health funds through HICAPS/EFTPOS. Please note that we do not accept American Express or Diners Club cards.

APPOINTMENT CONFIRMATION

A confirmation SMS will be sent to your registered phone number the day before your appointment. If you do not have a mobile number, we will contact you via your landline.

MISSED APPOINTMENTS

We understand that unforeseen circumstances arise, but missed appointments mean lost opportunities for other patients. If you are unable to attend, please notify our reception team as soon as possible. For after-hours cancellations, you may leave a message on our answering service.

SPECIALIST TREATMENTS

We provide advanced treatments, including:

  • Swift Microwave Wart Therapy

NAIL SURGERY

Nail surgery is available at Leading Health Podiatry. A follow-up redressing appointment is required the following day, with additional check-ups until the nail has fully healed.

BUILDING ACCESS

Our clinic is easily accessible, with a spacious parking area. The entrance is located at the front of the building, next to Modbury Medical Centre.
